Privacy Policy

This policy describes the types of information that Care Coordination Systems, LLC
(“CCS” or “We”) may collect from you or that you may provide when you use the
Community Health Record applications (“the CHR Applications”) and associated
software in the course of your employment, and our practices for collecting, using,
maintaining, protecting, and disclosing that information. This policy applies to information
we collect when you use any of the CHR Applications, and in email, text, and other
electronic messages between you and your employer and CCS.
This policy does not apply to client information entered by you into the CHR Applications.
That information is protected by law and by Business Associate Agreements between
CCS, your employer, and the Community Hub with which your employer is affiliated (“the
Community Hub”).
Please read this policy carefully to understand our policies and practices regarding your
information and how we will treat it. By accessing or using the CHR Applications, you
agree to this privacy policy. This policy may change from time to time (see Changes to
Our Privacy Policy below). Your continued use of the CHR Applications after we make
changes is deemed to be acceptance of those changes, so please check the policy
periodically for updates.
Information We Collect About You and How We Collect It
CCS collects several types of information from and about users of the CHR Applications,
including:
information by which you may be personally identified such as name, e-mail
address, or any other identifier by which you may be contacted online or offline
(“personal information”);
information that is about you but individually does not identify you, such as the
name of your employer and the information which you enter into the CHR
Applications; and
information about your internet connection, the equipment you use to access the
CHR Applications, and usage details.
We collect some information directly from you when you or your employer provides it to
us, and other information (such as information about your computer and internet
connection, including your IP address, operating system, and browser or tablet type)
